Site Evaluation / Soil Testing

To have a well site evaluation and/or septic soil testing performed, you will need the following:

• Completed Well Site Evaluation / Septic Soil Testing Application (Fillable) (PDF) including your 10 digit parcel identification number.
• Boundary/Grid Drawing Site Evaluation Grid (blank) (PDF) of where you like to test.
• Your drawing should include: Lot lines, major structures, underground utilities, etc.
  • Replacement systems: Need to mark where existing system currently is and the desired location for the new system. 
• Fees
  • We accept cash, credit or check.

You can submit an application and forms via email to EHealth@co.jackson.mi.us or fax (517) 788-4616. You will then need to call our office to make a payment with a card over the phone, visit our office, or mail your information to:

After you have applied and paid for your evaluation, we will give you the phone number of your sanitarian. You will be responsible for calling and setting up an appointment to come out and conduct your evaluation. Please note that soil evaluations must have a back hoe and operator on site at time of appointment.
