Thank you for your interest in receiving the COVID-19 vaccine. We no longer require that people leave their information with us on a survey. We have reached a point in the Vaccination Process where we are sharing our clinic signup links publicly; When scheduling please remember the following items:
Pfizer vaccine is a two shot series, 21 days apart and is allowed for 16 years and older
 Moderna vaccine is a two shot series, 28 days apart and is allowed for 18 years and older.
