We need your help! We are conducting a State Health Assessment Survey with people who live across Michigan. This anonymous survey asks Michiganders what could make their community a healthier place to live. If you are able, we are hoping you can help us by taking the survey and sharing the survey with other people and encouraging them to take it also. The survey should take no more than five minutes to complete.

The Purpose of the Survey is to Capture responses from Michiganders from across all of Michigan to identify the greatest health needs that currently exist in the state.

• Your experiences affect your health. Tell us about how you work to stay healthy and challenges you face. We want input from people around the state so we can see how health needs vary across Michigan communities.  
• Is your community a healthy place? Let us know how your community has supported your health or made it difficult for you or your family to be healthy. 
• We know that where you live can affect your health and wellness. Tell us what you need from your community to stay healthy. 
• The survey should only take five minutes or less of your time but is important to help us understand how you view health in your specific community.
