    • Jackson County Health Department staff will provide Medicaid Outreach to:
      • Increase public awareness of programs and services available to Medicaid-eligible and potentially Medicaid-eligible children and families.
      • Connect Medicaid-eligible and potentially Medicaid-eligible children and families to healthcare resources in the community to ensure appropriate care is provided to reduce healthcare costs.
JCHD supports the Medicaid program through multiple outreach activities including:
  • Coordinating and presenting information about Medicaid and Medicaid covered programs through media resources, health fairs and community events; 
  • Facilitating Medicaid eligibility determination by assisting potential Medicaid eligible individuals in applying for Medicaid benefits;
  • Collaboration between JCHD and other agencies to assure the delivery of Medicaid-covered services; 
  • Referral, coordination and monitoring of Medicaid services; and
  • Medicaid-specific training on outreach eligibility and services
